Insurance Information

Insurance Accounts:

This office will try to verify and determine your insurance benefits. However, we require that a portion of the bill be paid at the time services are rendered. This portion is based upon an estimate of your deductible and coverage. We realize that insurance is complex and difficult to understand. Our office will do everything possible to help you make the most of your insurance benefits. If we are working with your insurance company you are responsible to forward explanation of benefits and payment as soon as it is received. If there is a balance this will be your responsibility. In the event that benefits can not be determined we will ask for full payment when services are rendered.

It is important that we obtain the correct insurance information, such as a copy of your insurance card(s). Please remember that your insurance policy is a contract between you and your insurance company. Also, be aware that some oral surgery procedures may not be covered under dental insurance, medical insurance, or the Medicare program.

Usual and Customary Rates

We are committed on providing the best treatment for our patients. Our procedure fees are based on the usual and customary rates for our area. Our fees are not based on your insurance companies determined usual and customary rates.

Missed Appointments

Cancellations cause disruption in our schedule and in our ability to treat patients as efficiently as possible. If you must cancel or change an appointment, please notify our office at least 24 hours in advance. If cancellations become habitual, additional appointments will be denied. Please help us serve you better by keeping scheduled appointments.
